Shepard's reaction here was inspired by of all things Buffy the Vampire Slayer TV show.
When Buffy's mother died unexpectedly in one of the seasons, she was in complete shock as she was walking through her house looking for the phone to call 911. In her state she vomited, I guess she was doing her best to keep it together her body started to come apart and that was the reaction I kind of wanted Shepard to have here.
Shepard always has to keep it together for all the other people around her and sometimes you just can't do it any longer
